ATTENTION ALL OPERATORS:
Effective July 1, 2004, all well logs including Five Inch (5") are required to be submitted
to the appropriate district office. Well logs should not be submitted to the Baton Rouge Office.

Electric logs checked
out of the Shreveport Office are due back to the Shreveport Office by
3:00 P.M. the same day. A maximum of ten (10) logs may be removed from
the office. Party signing out log(s) is responsible for proper care and
safe return of the log(s). Request(s) to keep log(s) overnight must be
approved by the district manager.

Clearance and Test Allowable Requests |
TO TEST AND/OR PRODUCE
A WELL FOR WHICH A UNIT IS REQUIRED OR FOR WHICH A UNIT HAS BEEN REQUESTED,
AN EMERGENCY CLEARANCE OR TEST ALLOWABLE REQUEST SHALL INCLUDE THE FOLLOWING
INFORMATION IN WRITTEN FORM ON COMPANY LETTERHEAD TO THIS OFFICE:
- A statement to
the effect that ALL monies/revenue GENERATED from the first date
of
production shall be escrowed and later disbursed based upon the unit
to be created.
- FOR EMERGENCY
CLEARANCE: Beginning and ending dates of test production with total
amount of liquid and
gas produced.
- FOR 90-DAY TEST
ALLOWABLE: Date of first production to sales, liquid and gas rates.
- Gas purchaser
and liquid transporter.
- Perforated Interval.
- Status of unitization
application.
- Attach completed
preliminary Form WH-1 with request letter described above.
NOTE: A MAXIMUM OF
THREE (3) CLEARANCES MAY BE GRANTED FOR EACH COMPLETION BEFORE FORM R-4
IS APPROVED. ADDITIONAL TEST ALLOWABLES BEYOND THE INITIAL 90 DAY PERIOD
MAY BE GRANTED AT THE DISTRICT MANAGER'S DISCRETION AFTER REVIEWING THE
STATUS OF THE UNITIZATION APPLICATION.
| Inactive
Well Report (Form Inactive WR-1) |
An INACTIVE
WELL is one that did not produce and was not used as a
service well in the six-month period preceding APRIL 1 and OCTOBER
1 of each year.
The Inactive
Well Report is filed semi-annually and is due to the district office
in
duplicate by the 25th of APRIL and OCTOBER.
All SHUT-IN
wells may be listed on the same report form, i.e. all fields, all wells,
ONE operator.
Double-space
between each field
report.
Wells classified
as "no future utility" shall be plugged in 90 days. Substantial evidence
of future utility may be required by this office to justify/maintain
a shut-in status.

- Spill Prevention Control and Countermeasure Plan |
ARE YOU UP-TO-DATE
WITH YOUR SPCC PLANS?
- Title 40 Code of
Federal Regulations, Part 112 requires a Spill Prevention Control and
Countermeasure Plan be prepared for each oil and gas production facility.
- Administered under
the authority of the EPA, these regulations require the operator of
the facility to prepare and implement a plan to prevent a discharge
of oil into the waters of the United States and to provide for rapid
response and clean-up should a discharge occur.
- The District office
has a copy of an informative booklet published by the EPA describing
SPCC Plans which may be copied; or you can contact the EPA direct at
1445 Ross Avenue, Dallas, TX 75202. (http://www.epa.gov/oilspill/)
- Also, the American
Petroleum Institute has a publication, API Bulletin D 14, 'Suggested
Procedures for Development of Spill Prevention Control and Countermeasure
Plans' which describes the preparation of these plans. (http://www.api.org/cat/)
- Just a reminder,
for any spill/discharge at an oil or gas producing facility, call
the
local CES or the District Office, immediately, then the National Response
Center at 1-800-424-8802 and within one hour call the State
Police at 1-877-925-6595 (both are toll-free).

- Oilfield Site Restoration Program |
Act
404 of the 1993 Regular Session created the Oilfield
Site Restoration Law and the Oilfield Site Restoration Commission.
The purpose of the program is to properly close and restore orphaned
oilfield
sites. Click

Island Oil Museum - Oil City |
Visit
the Caddo-Pine Island Oil Museum, housed in the city's old railroad depot,
featuring an oil derrick and a variety of early oilfield equipment and
boomtown photographs. The
museum is located off Hwy 1 in Oil City near Caddo Lake, site of the 1911 "Ferry
Lake No. 1" well, one of the world's first over-water discovery wells.
