Current Well Owners

General Information

For all current well owners, please notify the Ground Water office when there is a change in the information we have on record on the original form. If there is a change of information regarding your well – it must be reported. Make the change(s) on the Water Well Notification form and indicate on the form that this is a change.

A water well notification form is necessary for the Office of Conservation to manage the state’s ground water resources.

Well owners may need to transfer ownership of a well or may need to make changes to the information that they have already provided to the Office of Conservation.

To transfer ownership submit a new form. You will need the GWR ID number from the original form to put on the new form.

To change or update your well information, fill out a new form and indicate the “change” on the form.

There are several types of wells:

  • Dewatering wells
  • Domestic wells
  • Frac wells
  • Industrial wells
  • Irrigation wells
  • Public supply wells
  • Replacement wells
  • Drilling rig wells
